Actinomycosis is a subacute to chronic bacterial infection caused by filamentous ,gram positive anaerobic to microaerophilic bacteria.It affects different body regions .However, primary actinomycosis of anterior abdominal wall is very rare and has not been described in standard text books of surgery. It is also difficult to get diagnosis preoperatively.
We report a case of 63 years old, previously healthy barber who began to notice a mild painful mass on right lower quadrant of abdomen for about 6 months duration. He denied of having any fever, trauma or abdominal surgical operation before.
Physical examination showed it was an extra abdominal mass. Echo guide FNA cytology revealed –it was negative for malignant cell. Abdomnal CT scan reported –it was a soft tissue mass measuring 3.6x3.2x3cm in right lower abdominal wall with intraabdominal invasion. Excision and biopsy was done. The mass had invaded to abdominal wall including fascia, peritoneum, and part of omentum. Tumour including abdominal wall fascia, peritoneum and part of omentum were resected. The tumour was not invaded to bowel wall. Pathological report revealed acute and chronic inflammatory pseudotumour like fungal element (Sulphur granule )of actinomycosis. We treated the patient with intravenous penicillin for one week and following oral penicillin for 2 months. Post operative period was smooth and the patient recovered completely. |
