Summary：A comparative study was under taken over a period of 3 years to evaluate the clinical and radiological outcomes between fixation methods using clavicular hook plate and tension band wiring fixation for distal clavicle fracture or A-C dislocation.

Material：82 patients (tension band wiring fixation was used in 40 cases and a clavicular hook plate in 42) were recruited for the study.The follow-up duration was 36 months. All patients were treated primarily.

Results：Five wires migration and two loss of reduction occurred in tension band wiring group. No implant fracture or loosening was observed in hook plate fixation group.

Conclusion：Hook plate is a relatively simple implant which achieves reliable result in terms of stronger fixation and less complications comparing to tension band wiring thus it is therefore recommended.
